Can we also get an RSVP gravity form?

Isaac Klausnser, Medstar Isaac can we link this RSVP form to the leadsheet with an "Events" tag? I know you typically set these up with Whitney what is the standard recommended setup here? Whitney is in a meeting right now but I wanted to get the ball rolling because of the short turnaround time.

Join us September 7th-9th for our Pop-Up Shop event. Three fun-filled days of shopping, prizes, specials, and our famous "Botox Bingo"!

Pop-Up Shop Events:

-Pick up your bingo card and play all month long to win a FREE Botox treatment.

-Stop by all week to shop our local vendors and pick up new workout wear or fabulous jewelry items.

-RSVP below to join our evening event on Thursday, Sept 9th from 5pm-8pm.

We will be serving hors d'oeuvres, cocktails, and raffling off a FREE CoolTone treatment!
There will also be goodie bags for the first 10 people to purchase a medspa treatment during the event.
