Also, on this text form we are doing could we try having WordPress send the email from the customer's email they input to the doctor could easily click reply to the email or would this cause issues of going into spam.

Also, can we put the customer email in the reply area so the doctor can easily reply to the customer who sent it.

When the doc gets a lead he wants to hit reply and have it go to the customer email that inputted and his two staff emails

This is excellent can you please test it on mobile and different devices. It it works make the adjustments to the live form. Also Make sure the leadsheet has an area that shows which services they are interested in also make sure high level tags the service so if they select multiple services they will have multiple tags
