Shane Quast, Medstar Shane I know you mentioned this in the chat. The SEO team changed the URL without checking if ads were going there and missed instructing to 301 redirect. It is really unfortunate, and hopefully won't happen again.

This is what the URL was changed to:
https://laserdocmd.com/coolsculpting-baltimore/

I was able to resolve the issue for forms not submitting.
