Hector - They only have one form with the title "Request Information" placed in the sidebar on all pages on the website which on my end is not working. Can you please check on your end?

You are correct that the form just stays loading and does not fill out at all.
How have these people not noticed this for so long?
In any case, we are doing their website correct?
So doesn't that mean we will have new forms for them?

Creating a new form wouldn't fix the issue as it's a theme JS issue. Can you pls confirm with Chris if we need to fix this issue on our end as fixing this js issue may take a couple of hrs?

If we are doing marketing for them - which we are - we will need to track leads. That means this has to be done. 
In any case, I will verify with Chris and get back to you.

Let's just make sure we are using a theme that is effective. 
Now if it's your professional opinion that this will have to be fixed regardless of the new form, then please do so.

Hector - yes, we can switch to a different them but that will involve more amount of work.
