I am seeing that this website and client are having the same issue with their pop ups that I found on Dr. Curby site. You enter the information but everything is coming up blank on the leadsheet. The only info showing is the date. 
Can you please look into it?
Also, please find the common denominator between these two sites so we can avoid it happening again.

It's because whoever set up popups for both of these clients used the same webhook that I created for Gravity forms.
