Here are the edits I made.

Here are the changes I made:

1. I applied the Meta description and Meta title from the document.
2. Edited the Webhook name of this form. I removed the “ Copy 1”.

3. I added a B&A disclaimer from the standard B&A disclaimer provided by Medstar.

4. I added a bottom space here. And change the font of the “ With so many…” paragraph to match the rest of the page.

6. I adjusted the font of the answer on the 2nd FAQs to match the other two.


7. Mobile Hero Button doesn’t match the GF forms. Changed this to “Get Coupon”.



I need confirmation from you on these:

-We can leave this like this, right? This section has “in SLC” added.
