I am removing everyone else from the task and reassigning to myself.
For botox, Mobile only populates 'Rancho' and 'Any location' to the leadsheet. Please check why it did not for Redlands and Hesperia. Additionally, the pop-up submission did not populate to the leadsheet.
On the app for botox, only Redlands and Hesperia tags were activated, not 'Rancho' or 'Any location'. Also, the pop-up did not generate to GHL for Botox when I submitted
For Potenza The bottom form for 'Rancho' does not populate to leadsheet, and this is what I see (1st image). Also, the source URL did not populate on the gravity forms, only the pop-up source URL populated(2nd image)

Usually above happens when we do too many submissions using same test data from a same IP address. This is a default mechanism of Gravity form to defend against the spammers. So whenever you don't get redirected to the thank you page there is a 95% chance the form didn't submit successfully. In the rest 5% it could be that you form got submit successfully but didn't got redirect due to technical issue in the backend.
I crosschecked by checking the entries and also in the zapier logs and couldn't find your test submission for the botox mobile submission for the Redlands and Hesperia.
I just did a test submission choosing Redlands as preferred location and it came to the leadsheet successfully with the correct location info.
I checked in the zapier logs and coulnd't find any your test submission from the botox popup. I could only find a test submission from Potenza service page popup.

I checked in the logs and it shows that it passed the correct info to the GHL. You can check in the tags row in the below screenshot but it seems you deleted this test leads from client's GHL sub-account so I am unable to check why it's not showing the potenza service as tag. I looked in the popup settings and all looks good. Delete all your existing data in GHL and then do a test again. I would recommend doing the same everytime you do a new test submission. For the location tags missing from it. It wouldn't show the location tags for the popup submissions because we don't have any location field on the popups.

I would recommend you delete your previous test lead from the GHL before doing another test submission.
Also testing the forms in the incognito mode wouldn't make much difference because GF checks for the IP address and the data while you make submissions. So to make sure all your tests gets submitted successfully. I would recommend switching to a different IP address and also use a different test data.
Also in the case where you test all the forms and popups on a website. I would recommend either giving a heads up to the client for all the test submission notifications or temporary disabling the client notification. I am asking this because in the past when some of our new devs did so many tests it bombarded client's inbox without any prior notification and they weren't happy about it.
