I just checked both the pop ups and the side forms and they are good to go. 
However, I am not seeing my tests show up on the leadsheet. 
Can you check to see if the side forms and pop up are hooked up?

Hector Mota, Operations Manager at Medstar Hector - I just did a test submission from the side form on this service page and it came successfully in the lead sheet. I also crosscheck in the backend to make sure and everything looks good.
