what exactly is not working on that landing page for Colorado?
I tested it out, both side form and pop up.
The only issue I see is that on the pop up, the LEAD SERVICE TYPE is not
showing up.
Isaac Klausnser, Medstar Isaac can you please fix that real quick?

I'm not so much worried about the popup as much as that the page never specifically said they have an office in Colorado Springs. No one is going to drive from CS to Denver and I think people are confused by the page.
