Paige it is a new website or have they only been making adjustments to the design layout? I will check connections and gravity form settings, hopefully, they haven't messed with that.

My word choice was too lax sorry! 
Please do check the connections, their web dev team didn't seem to consider many standing choices we had in place.

Hi Alyssa and Isaac   I went ahead and test all the gravity forms across the whole website. All are working well aside from the Hydrafacial page. 

For the Hydrafacial page - after signing up on all the gravity forms, I wasn't taken to the correct confirmation page. I was taken to this image instead and it didn't went through the leadsheet as well.
