I found an issue with the side forms on this client's website not showing up on the lead sheet.

Issue on side form - https://jwalkerwellness.com/plasmafibroblast/ 
https://jwalkerwellness.com/permanentmakeup/
both are not working. 
Can you please check on this right away as we have campaigns running to these sites.
