there seems to be an issue in this client's acct. A lead came over without a source URL and name. On the leadsheet it has this information but not in GHL.
The one that is marked with the red in the below screenshot came from the contact form that you are also seeing on the leadsheet.
But the other one that is marked in yellow came from inbound sms.
Although the automated message went through successfully to this lead when she submitted the contact form but due to some unknown reason GHL saved the wrong number in the database and therefore once she replied back it considered her a new lead and thus added her again in the contacts.
"By default if a new lead send a sms to the GHL tracking number it will get added as a new lead."
How would a new lead send an inbound sms thats not linkd to a conversation associated with the initial auto text? Is it a glitch- the saving of the wrong number?

This new lead showed up on the app on my phone but not on the desktop app.
CORRECT
She received the initial auto text successfully but after that somehow Highlevel saved the wrong phone number for her in their DB so now when she replied back to the initial text, the GHL system considered her as a new lead and thus added her as a new contact.
