Selena - I just checked and that's how both of the forms look on my end i.e without the message field.

Also, all sub-domains tied to this domain - mysculpt.net are hosted on our Siteground server so in order to purge their cache. You will have to install SG optimizer plugin.

It has the wpengine tab in the backend because we copied this template from the website hosted on wpengine.
