I have a high priority task for you.
Clients Bodymorphmd, Aura Derm and Cliffside have "Am I a candidate" form on their site created through Typeform.
Each Type form that is on their page should be separate - We started one template on Typeform. We need to make sure those have been duplicated.
We don't want all the doctors getting the same information on the typeform.
We need to test these out and see if they go into the lead sheet of each client.
If they don't, please let me know so I can speak to Isaac about it.
Login to typeform to check it out. Make sure that there are separate Am I a candidate forms and they are all hooked up properly.

Since Jonnamae was not available to do this task, I went ahead and began working on it. I found an URGENT issue we need fixed as soon as you can. This is HIGH priority so please fix right away.
When I tested the Emsculpt AM I A CANDIDATE page for https://sculpt.cliffsideskinandlaser.com/am-i-a-candidate/ the lead went to sculpt AURA DERM lead sheet.
Please make sure that this is corrected by adding Cliffside leads to the right sheet and removing them from aura derm's.
The BodymorphMd.com Am I a Candidate form are working as it should.

The problem still exist. I just went to
https://sculpt.cliffsideskinandlaser.com/am-i-a-candidate/ and filled out a form.
I did not see the lead go to their sheet. Instead, it is still going to Aura derm's.
Can you please check again?
I see the lead going to the Cliffside now.
The LEAD SERVICE TYPE tab is supposed to say EMSCULPT as it does on AuraDerm. Can you please add that as well? Thanks.
