✔ Make sure CoolSculpting pages have 2 separate Gravity Forms

We need to go through and find all the CoolSculpting pages that have two forms on one page. Many of the CoolSculpting pages have two of the same gravity forms. This can cause problems when they do not fill them out properly. Can you go through and make a duplicate of the top form. Leave everything the same (thank you page, notifications, etc.) and then insert the duplicate form on the bottom. Then I would like you to test one of the forms enter your name as test-lead-ignore. Note the thank you page, make sure the thank you page as the same content as the treatment page with the forms taken away and marked no-index. Make sure you get a welcome message and if you do copy it into the client audit tab found here.

I also added some other audit items we can perform. Such as testing the popup, are the sites on jupiter have the standard custom form or a gravity form with the special code issac told us to put in.
