This will be a good project for you. Login in to my typeform and look at some of the templates. Basically it is a questionaire.

First, we will get them to answer which parts of their body they want to target, then ask them for their  name, email, phone and ask them if they had any surgeries.

Here is an example of the typeform. Typeform does not have great images to choose from, if you want I can upload some of our photos from shutterstock to make it look nicer.
Also I don't think we should ask if they have had any previous surgeries on this questionaire, that is something the doctor or even front desk can answer once they get the call back.

Melissa Neil, Medstar Melissa  - Your making good progress on this. My only suggestion is tighter shots of the body parts. Meaning if its abs it should focus on abs and not have a lot of background noise.
