I found two issues with the lead sheet for this client.
One, it appears that the POP UPS for the hydrafacial page are not connected to the lead sheet. I filled out one and did not see a lead emerge.
Second, the time is wrong format on the lead sheet. This is how the day is coming out =- 44077.8663541667
can you please fix that as well?

I can also see you test lead in there. Date is also appearing in correct format.
