I am seeing an error with the confirmation page on the emsella side form.
First, it is not populating on the leadsheet.
Second, it goes through but scrolls to the bottom, CSS issue on top (See screenshot)

Can you please look into it?
I also found an issue with the side form on the contact us page. I found it odd that I did not see any leads from there.
So I tested the contact us page. Leads are not populating even though confirmation page is showing.

BTW, look at the confirmation page. It shows a side form still.
That should not be that way correct?
We should have Jonnamae or Kristabel fix that as well. ?

I checked in the leadsheet and was able to find your test lead from this page in there.
I have now removed the form on the confirmation page.

There must have been a delay on the side form because it was not populating when I checked.
In any event, glad you fixed the other issues as well.
