Yes sir. Already look in the settings. Everything looks good in there.

So in order to inspect further and find out what's causing the redirection to not work properly on that page.  I need to check the error logs and for that I'm going to need SFTP details.

Ok I asked developer. Are you able to remove the Request an Appointment form from that page. It should only have the coolsclupting gravity form

Yes I also noticed that but in order to remove it only from source of this page. We need to create custom template as if I remove that from the widgets area. It will get removed from all other pages that using the same default template.

Isaac  - they did not give SFTP but they did say they cleaned up the errors. could you please look into it. 

Hmm..I can go ahead and inspect for the redirection issue + create new custom template for above pagesusing file manager plugin directly from wp-dashboard. But it would be little risky.

The problem with editing php files directly from dashboard is if some php error occurs. We can't revert it back from dashboard. WP doesn't allow that. We must need access to cpanel/ftp in order to revert the changes.

Could you hide it again just on that page without any issues? I am still having issues getting SFTP access from the developer. I am also noticing the redirect still doesn't work

I am going to need some time to look into form redirection issue as it's little tricky. I will make a copy on my local server and will do couple tests there. So for now. I have hide the form appearing above coolsculpting form in the sidebar.

Just tried to create new file on the server for the template. Got "permission denied" error.
