To block spams on our client sites we use (Akismet + Google reCaptcha InvisibleV2 + hidden Honeybot field) On most client websites these three anti spam techniques stops the spams completely. 
On some specific client websites if they still see some spams coming in we go further and block .ru email submission + block any URL submission in notes/comments field.
