I am getting a 404 when I fill out the pop up on the

Thank you for taking care of this. I see the pop up issue working. However, I am having problems with the side forms.
I filled it out and the phone number would not go through. I kept trying and had to add my own number and then I got a critical error..
I see that has also been fixed now, so thank you.
One question. I am seeing that some of the forms or most of them - do not have the (111)(222)(3333)
option on the phone number line.
What mean is that option was not showing up on the dermal fillers page before and now it is after you fixed it.
But the other pages do not show that. It is just a normal slot as just like the one where you put your email and name.
Does that make sense? And if so, should we fix it?
