Hello Dr. Spitale,

How are you?

As you know, Google ads is now requiring that all accounts serving ads in the US are verified. This includes providing documentation which demonstrates that the business is eligible to run ads in the USA. 

The verification is for your Google ads account -  Spitale Laser - 132-405-1534. The verification process needs to be finished by October 14th. Otherwise your ads will be paused and the account could be suspended indefinitely. 

The good news is that I have already begun the first part of the verification process for you. I have now verified the business part of the account. However, we still need to verify your organization. You can start the 2nd part of the verification process yourself or you can send me the documents. I can then take care of the rest of the verification for you. If not, then we can set up a Zoom meeting with Vania & I so we can help you take care of it.


You will need to provide Google Ads with ONE of the following documents:

A document for the organization 
  •  A letter or notice issued or stamped by the IRS, such as CP 575 that confirms your Employer Identification Number (EIN) 
  •  A copy of the organization’s Form 8871, 8872, or 990 from the IRS website 
  •  A state-issued certificate of business registration or incorporation 
  • Your organization’s most recent SEC filing, including Form 10-K, 10-Q, or 8-K 
  •  A business credit report from Experian, Equifax, TransUnion, or Dun & Bradstreet 
An ID to verify it’s really you ** IDs must show all 4 corners **

  • Driver's License - Front & Back - High Resolution & in Color
  • State ID - Front & Back - High Resolution & in Color 
  • U.S. Passport or Passport Card
  • Green Card
I am attaching a blank W-9 form just in case so you can see what it looks like and fill it out. Since we helped to create this account for you, I have already started the 2nd process.

Thank you for sending over the ID and documents. 
As I stated earlier, I would not recommend that you do not send out your Social Security number to Google ads since 

we are not sure who exactly will see this information. 
Perhaps I am being paranoid, but it is better to be safe than sorry.
Having said that, I edited the photo so it only showed your EIN number and not your SSN. 

I then went ahead and made some other changes to the account and submitted it to the Google ads team. 
Guess what? It WORKED!!!

I am happy to report that your Google ads account has now been verified. ((See 2nd screenshot))
