Sydney - Yes. I already fixed it but I would recommend we notify the dev who set up the forms on this website about this error so that they can keep this thing in mind while setting up the forms on any new client website. This is a very important as it directly affects the customer.

Thanks Sydney & Isaac, I am going to keep this task open until we inform the correct developer about this. Thanks for catching it and fixing it. I am reassigning to myself so it doesn't get lost. This is important.

So basically while creating duplicate of the forms for the new services. We need to make sure we update the service value in the hidden field and also in some cases (mostly the old clients) We need to make sure to update the form notification settings as well.
