Following the completion of client {Surface Medical Aesthetics/ Della Chiaie - www.surfacemedicalesthetics.com} and their website  (https://surfacemedicalesthetics.com) setup, began to check on pop ups, side forms and other forms to make sure they were working correctly. And that they were populating on the leadsheet. Found some issues though, notified team.

I am seeing that when I fill out the side form on this page
https://surfacemedicalesthetics.com/botox/
that it just loads and loads.. but does not go through. If you by mistake then click on any part of the page, it goes away... 

Also the bottom form is giving me problems. When I hit submit nothing happens. I had to hit enter like 4 times before it went through.

Hi Hector Mota, Operations Manager at Medstar Hector - I just checked but couldn't replicate the loading issue on my end. The side form got submitted in less than 5s. It could be due to a temporary server issue as this website is hosted on a very low resources server. Please check again on your end and let me know. Thanks.
