Leadsheet Issue - Changes

I am seeing that this client's leadsheet has had some changes made to the way leads populate.
First, the names of the leads are no longer showing, only the emails. 
Second - this is the biggest concern - the leads are no longer showing the dates that they came in on. 
These changes seemed to have taken place after April 1st sometime. 
Can you please look into this when you get a chance?

I am not seeing that it has been fixed. I just filled out a lead and it is showing up the same way; no date or first name or last.


Also, on this page, https://vmedspatx.com/laser-facials-dallas/
The pop up is not letting me put any info on it. Where it says enter your information, I cannot enter anything.
Can you please check that out?

Okay, so now after reloading the page, I am seeing the pop  up show up entirely Isaac.
That is weird so you may need to check make sure nothing is off.

I have now created a new tab in the leadsheet and moved all the old leads to the old tab this is because I had to create two separate columns for the NAME information.

For the popup. I checked in different browsers both in normal and private window but couldn't replicate the issue. It could be due to a temporary glitch on the Hubspot API that the form didn't appear on your end in the first go. We are using Hubspot embedded form in the popups on this website.

You said you created a new tab correct?
However, I am still not seeing the dates come up on the column "I", only numbers.
Are those the Epoch number? Or dates coming out in another format?

The popup issue seems to have been fixed now.

You mentioned that you would create a video for me in that task to show me how to convert them. But I do not see it. 
Also, is that the only way to do so? Does the date have to come out that way from now on?
