these all look like fake leads that may have come in from either the hack to the website or, from someone on our team continuously testing the microneedling page: https://willow-aesthetics.com/microneedling/

Did someone on your team use these other aliases as well?

Clients will get tons of fake leads coming in every day. It doesn't mean it is because their page was hacked. Just spammers who do it. 

We only need to be aware of the ones we submit as test leads. 
I am almost certain no one on our team or Shane's filled out any of the following:

There's really no reason to tag Chris in this. Please save BC3 tags for Chris for real important things. 
As I said in the email, this is really not a big deal. Clients get spam leads all the time. Daily. 
I am more concerned about the fact that there isn't another lead on the sheet today. And I cannot test anything for fear that this client will make a big fuss about it.
