please do NOT take his suggestion of making the secondary keyword "large heavy breasts" lol- i have informed the client that this would put us on the porn search engine instead of google search engine. Let me know if you see any other edits that don't make sense for SEO. perhaps we can add "breast lift" as a keyword as well?

Mammoplasty to reduce breast size
Just double-checking where did you get the photos that are currently on the page? Also we need to add something like this example to cover the breasts, otherwise, it will shut down the ads. The gallery like this to be covered as well if adding.
I updated the section on how to view the before and after photos.

Is it possible to add that 18+ portion with boxed border to look similar to how the FAQ drop-down looks with white padding surrounding, instead of having the gray go full-width? I think it would look nice to have it more consistent.

If we mark the page as no-index on the go website it wouldn't rank. The best way here would be to set up 301 from the go website to the main website. This will tell the google bot. It's the same content but have moved to a different domain now.
So to confirm. Client wants to have the page rank on their main website right? If yes. Yes. We can mark the page no-index and remove from the menu on the go website and add it on their main website with indexable status.

They appear to be different from the other. If so, then you can mark both of them indexable. However, we would always defer to their main site; especially for SEO clients.
Crawling and indexing are two different things, requiring two different steps.
When you have a paid ad, the page cannot be blocked from crawls.
This is irrespective of whether or not the page is being blocked from being indexed.
Coincidentally, a page can be crawled even though it is not indexed or is prevented from being indexed. The only thing Google ads worries about is that there are no policy issues on the LPs or the ads themselves.
The best way to look a this is that a page is crawled by Google to be inspected to make sure it complies.
A page is indexed only to be marked as ready for those looking for that in future searches.
Having said all of this, it is always better to have LPS marked as indexable for trust issues.
