To answer your questions: 1. Leave the form as is. Can you just center the content beginning with "What causes urinary incontinence"?
2. Leave the (client) and (number) as is. 

Further edits:

I would like to add in red a title below the hero that says "Emsella Marketing: An example of an ideal page layout"

Then I would like to add a red box with a lightbulb for some comments on some sections.

These would be placed above the benefits section and above cost to begin. These "comments" will be added inside the box.

Above the "Emsella Benefits" section:

A way to generate Emsella leads for your practice, is by listing the benefits at the top.

Above the "cost" section: 

The keyword Emsella Cost is very helpful to have on your website because it's a term that is searched for a lot on Google.
