I believe you only have the side forms - https://navacenter.mysculpt.net/botox-dysport/
hooked up to the leadsheet. 
We also need the other pages, popups.

Hector Mota, Operations Manager at Medstar Hector - I just checked and all popups and forms are already hooked up on this website. I checked in the client's lead sheet and was able to see your test submissions in there.

The forms are now connected because Jonnamae hooked them up to Zappier in another task I assigned to her. 
When Chris and I tested them yesterday, they were not hooked up. Those tests were only for the side forms. The ones we did for the pop ups were not populating. 
In any event, they appear to be hooked up now.
