page Chris would like the following changes made. 

He said the forms look bad. I know we get more conversions when we take out the notes section field but now it looks super bad.
Jonnamae Jumawan, Medstar Jonnamae or Kristabel, Medstar Kristabel can you please update the form? 
Chris would says 

I think we should at the very least put the name of the service in the title on two lines and maybe a slight background around it.

Also he thinks we should have the before and afters on emtone and emsculpt up higher. and maybe put the Emsella machine on the page. It just looks flat, I know we rush these things but we should have a team come in and polish them once they are live.

#1 Lorem ipsum text in the footer on the mobile version

#2 Ajax is not enabled on some forms on the website - home bottom and contact us page.

#3 Mobile menu hamburger icon looks a little off due to one extra different size bar.
