There are some things that still need some attention:

1.  Blood Draw instructions Form didn’t get replaced

2.  BHRT Questiionnaire Men doesn’t work

3.  HIPAA-Leave only one that works and delete others

4.  Move COVID all the way to the top as all patients need to sign that, can you put it above BOTOX?

5.  Consents are all repeated at the bottom

6. Please put pellet fees at the top in blue link

7. HCG - Make H blue
